Tools for Advanced Planning

Patients and loved ones too often are asked to consider hospice during a time of emotional crisis, which means it's that much harder to make decisions. Family Hospice and Palliative Care encourages you to "have the conversation" with your loved ones about your wishes for care - and theirs. Take the time to talk about your options and put your advance directives in writing. Hospice helps ensure that your wishes are respected. These tools will help you be prepared. Five Wishes

The "Five Wishes" document gives you a way to control something that is very important - how you are treated in the event of a serious illness.
